In reviewing and completing this form, I hereby give my child(ren) permission to participate fully Science @ Home virtual programming through the Connecticut Science Center.
I understand and agree that:
• I retain full supervisory responsibility for any and all activities that happen as a part of the Science @ Home experience.
• I will not expect STEM staff to provide supervision for my child while they are participating in virtual activities or completing the suggested activities independently.
• I understand that participation in these events is at my and my child’s own risk. Some of the activities include small items that are potential choking hazards for young children, the use of tools like scissors that may require adult supervision and food or food-like items that might be harmful if ingested. While measures are being taken to manage any risk, there is still the risk that something unexpected may happen. I understand that I should monitor my child during these activities. If my child gets hurt while participating in any virtual or recommended Science @ Home activity, I will assume full responsibility.
• The Connecticut Science Center will be recording the live webinars. My child(ren) may be in either a screenshot and/or video recording. These may be used by the Connecticut Science Center for current or future advertisements. I understand that I will be notified which sessions may be recorded ahead of time and if I do not wish for my child(ren) to be in the picture or the recording, it is my responsibility to turn off their camera and/or mic. Leaving either of those two on is implied consent.
• If my child or I upload any photos or videos of my child, our family and/or their activities, I am giving permission for them to be used and shared with the other Science @ Home participants. Anything posted to the STEM in a Box digital platform will have to be approved by Science @ Home staff.
• I understand that the virtual programs that my child participates in will be closed and will only allow registered participants and approved staff to participate.
• For all virtual programs, like a Zoom class, I will ensure that my child is appropriately dressed and that the computer and camera that they are using does not show inappropriate things in our home. If staff notice something on your child’s video that staff determines is not appropriate, they will turn off your child’s video until the issue is resolved.
• My child(ren) may communicate in private media set up for two-way classes and discussion groups, such as a zoom class or chat. These will be facilitated by at least two approved adults with at least two children and one approved adult in each breakout room. The overall facilitators may join any breakout room at any time.
• Appropriate behavior for participants will be expected. Staff will review appropriate and inappropriate behaviors at the beginning of each virtual event. Bullying, including cyberbullying, during a session or outside of a session is not appropriate. Issues and consequences will be addressed with the child and parent including and not limited to temporary or permanent removal from the program.
• It is my responsibility to provide any equipment needed for virtual programs, such as a computer, tablet, internet access, headset, earphones, microphone, etc. I also understand that any damage to said equipment is the sole responsibility of the owner of said equipment. I will not hold Connecticut Science Center responsible for any damage that may happen during Science @ Home activities.
